Telehealth Consent

This Telehealth Informed Consent explains what it means to receive care through the InstantMD platform. Please read it before your first visit — you will be asked to acknowledge it when you book.

The short version

Telehealth brings your clinician to you by secure video and messaging. It is voluntary, it is not for emergencies, and every treatment decision belongs to your licensed clinician.

Section 1

What telehealth is

Telehealth means receiving health care services from a licensed clinician remotely, using secure video, audio, and messaging instead of an in-person office visit. Your clinician can evaluate you, discuss and order treatment, prescribe medication where clinically appropriate, and coordinate your ongoing care — all through the platform.

Expected benefits

Care from home without travel or waiting rooms; timely access to licensed clinicians; continuous access to your care team by secure messaging; and records of your care kept in one place.

Section 3

Possible risks & limitations

A remote visit does not include a hands-on physical examination, which in some situations can limit evaluation; your clinician will tell you if your care requires an in-person visit and will help direct you appropriately. Technology can fail — video or audio may disconnect or degrade; if that happens, we will reconnect or reschedule your visit at no disadvantage to you. As with all electronic systems, security safeguards — while strong — cannot remove every risk.

Alternatives

You may seek the same services in person from a provider of your choice at any time. Choosing telehealth is voluntary.

Section 5

Your rights

You may withdraw this consent and stop telehealth care at any time, without affecting your right to future care or your Medicare benefits. You may ask your clinician questions about your care, the technology, or this consent at any point. You have the right to access your medical records, as described in our Notice of Privacy Practices.

Section 6

Privacy & recording

Your visits and messages are protected under HIPAA and our Notice of Privacy Practices. Visits are not recorded by us.

Section 7

Emergencies

Telehealth is not for emergencies. If you believe you are experiencing a medical emergency, call 911 immediately or go to the nearest emergency room. In a crisis, do not wait for a message reply or a scheduled visit.

Section 8

Prescriptions

All treatment decisions — including whether any medication is prescribed — are made by your licensed clinician based on their professional judgment, your health, and the applicable program’s criteria. A telehealth visit does not guarantee a prescription. If a medication is prescribed, it is sent electronically to the pharmacy you choose.

Section 9

Care team & supervision

Your care may involve physicians, nurse practitioners, and support staff working together, operating under the clinical direction of our Chief Medical Officer and in accordance with state supervision requirements.

By checking the consent box, you confirm that: you have read and understood this consent; you have had the chance to ask questions; you understand the benefits, risks, and alternatives of telehealth; you understand telehealth is not for emergencies and that no specific treatment or prescription is guaranteed; and you voluntarily consent to receive care through the InstantMD platform.
